CHAPTER 15HEALTHCARE ADMINISTRATIONIn the Medical Department, proper recordsadministration is very important. We are charged withadministering not only routine personnel records, butalso clinical records that may affect the rights andbenefits of patients and their dependents years beyondretirement or discharge.As a Hospital Corpsman, you could be assigned toor responsible for the administrative affairsconcerning inpatients or outpatients. This chapter willprovide information on the function of healthcareprograms you may be involved with or responsible for.We will also discuss the legal implications in medicalcare, including the various aspects of consent, incidentreports, and release or nonrelease of medical informationunder the Privacy and/or Freedom of InformationActs. Further, guidance concerning your relationshipand interaction with law enforcement personnel andthe legal community will also be outlined.PATIENT ELIGIBILITY FORHOSPITALIZATION ANDNONFEDERAL CARELEARNING OBJECTIVE: Recognize thepolicies and procedures for DEERS,CHAMPUS, and TRICARE.The fact that a person seeking treatment is or wasconnected with the federal government does notautomatically entitle him to treatment at a navalmedical treatment facility. A number of factorsdetermine eligibility for certain types of medicalattention and the source and amount of remunerationfor that treatment. The following section deals witheligibility verification by presentation of a valid IDcard and utilization of the Defense EnrollmentEligibility System (DEERS). Further guidance can beobtained by familiarizing yourself with the followingsources:NAVMEDCOMINST 6320.3, Medical andDental Care for Eligible Persons at NavyMedical Department FacilitiesNAVMEDCOMINST 6320.18, Civilian Healthand Medical Program of the Uniformed Services(CHAMPUS) RegulationNAVMED P-5020, Resources ManagementHandbookDEFENSE ENROLLMENT ELIGIBILITYREPORTING SYSTEM (DEERS)The Defense Enrollment Eligibility ReportingSystem (DEERS) was developed to improvedistribution and control of military healthcareservices. Additionally, DEERS was implemented toassist in the projection and allocation of costs forhealthcare programs and to minimize fraudulenthealthcare claims. Navy medicine’s eligibility for careinstruction, NAVMEDCOMINST 6320.3, providesguidance as to who and under what circumstancesmembers can receive medical and dental care at NavyMedical Department facilities; the extent andconditions under which such care may be provided;and the collection process to pay for that care.Enrollment for all seven uniformed services (i.e.,Army, Air Force, Marine Corps, Navy, Coast Guard,Public Health Service, and National Oceanic andAtmospheric Administration) is accomplishedthrough completion and submission of an Applicationfor Uniformed Services Identification and PrivilegeCard, DD 1172, for a member’s dependent. When anew ID card is obtained for the dependent, themember’s DEERS data is updated online. If problemsexist within a patient’s database, active duty personneland their dependents must be referred to the sponsor’spersonnel office. Refer all other beneficiaries (e.g.,retired personnel and their dependents) to the nearestpersonnel office.Direct Care System ProceduresIn addition to providing authorization to standardmedical care through inclusion in its membershipdatabase, DEERS now includes a dental policy basedupon beneficiary information (versus the previouspolicy based on sponsor information). This change inpolicy occurred in part because of the increasedaccuracy of the database as well as the percentage ofpersonnel enrolled.15-1
