Medical surveillance examinations assess thehealth status of individuals as it relates to their work.Although these exams are not physical examinationsas described in this chapter, they are actuallysurveillance examinations that produce specificinformation with regard to an individual’s healthduring actual or potential exposure to hazardousmaterials (i.e., the Asbestos Medical SurveillanceProgram [AMSP]). Specific guidance on the AsbestosMedical Surveillance Programis provided inOPNAVINST 5100.23. Another example of a medicalsurveillance program is the Occupational NoiseControl and Hearing Conservation Program.Personnel who work in areas of high sound generation(e.g., flight deck of carrier) must be evaluatedperiodically for hearing loss. Specific guidance on theO c c u p a t i o n a l N o i s e C o n t r o l a n d H e a r i n gC o n s e r v a t i o n P r o g r a m i s p r o v i d e d i nNAVMEDCOMINST 6260.5 and OPNAVINST5100.23.MEDICAL BOARD EXAMINATIONS ANDREPORTSMedical review boards are the single mostimportant factor in determining fitness for duty intoday’s Navy. Medical boards are convened andreviews are conducted to determine the variousdegrees of fitness for military service. Local(re)evaluations are scheduled to assess patientprogress and length of limited duty, or need for aformal evaluation at NAVPERSCOM or the PhysicalEvaluation Board (PEB). The following examplesillustrate the legal guidelines, requirements, and jobdescriptions for the different types of medical boardsand describe the duties of the personnel responsible.Abbreviated Temporary Limited Duty (TLD)Medical Board ReportThe abbreviated board report is used only when amember is expected to return to full duty after anadequate period of treatment. Processing time shouldnot exceed 6 working days, and under mostcircumstances, the report should be completed in 3working days. The board report is a local action takenby an appropriate medical or dental officer and doesnot require external departmental review byNAVPERSCOM. The form (NAVMED 6100/5) usedfor this report is a multi-copy form. It is a vehicle forrecording basic medical findings, plans, andexpectations in terms of prognosis and length ofmedical restriction of activity. It also provides forparent command acknowledgment and comment. Thisform serves as excellent input for the “puttingperformance into practice” form in the member’shealth record; however, it is not a substitute fordetailed documentation of conditions in the member’shealth record. The Abbreviated TLD Medical BoardReport (NAVMED 6100/5) is to be used when all of thefollowing criteria are met:The member is enlisted in the U.S. Navy orMarine Corps.The member suffers from an uncomplicatedillness or injury which makes them temporarilyunable to fully perform duties to which they areassigned or expected to be assigned, but willmost likely be fit for full duty after an adequateperiod of treatment not exceeding 8 months.The member’s health or clinical record containsadequate documentation on the nature andcircumstances of the illness or injury, its course,prognosis, and treatment.Patient (Re)evaluation/TLD DurationOnce a member has been placed on TLD, thephysician, dental officer, or Independent DutyCorpsman (IDC) (when in an independent operationalduty environment), willconduct a detailed treatment/rehabilitationassessment and develop a treatment/ rehabilita-tion plan;ensure follow-up evaluations every 2 months,documenting at each evaluation objectivefindings of continued unsuitability, progresstoward recovery, findings and recommendationsof specialty evaluations, modifications to thetreatment/ rehabilitation plan, and prognosis forreturn to worldwide assignability; andobtain approval from NAVPERSCOM(NPC-821) or CMC (MMSR-4), via the patientadministration Limited Duty (LIMDU)Coordinator for periods of TLD less than 8months, or via a formal board (NPC-821 orMMSR-4) for periods longer than 8 months.Approval must be obtained via formal board if the initialrecommended period of TLD exceeds 8 months, and thetotal period a member can be on LIMDU must notexceed 16 months.11-4
